Health care costs for a family of four with employer-sponsored health benefits in 2025 are projected at $35,119, up 188% from $12,214 in 2005, according to the 2025 Milliman Medical Index (MMI). The MMI is based on the cost of an employer-sponsored preferred provider organization (PPO) plan, plus out-of-pocket costs.
