This presentation took place at the 2025 OPEN MINDS Service Excellence Institute. Attendees heard from Monica Dedhia, LCSW, Director Of Community Health, Children's Institute and Don Taylor, LCSW, Chief Executive Officer, Catholic Charities Of Santa Clara County. They discussed taking a person-centered approach to address not only a patient’s clinical needs, but that are also assisting with social needs to thwart chronic illnesses and poverty.
