A Telemedicine ‘Transitions Of Care’ Clinic For High-Risk Consumers Reduced 30-Day Hospital Readmission Rates
A telemedicine clinic that provided virtual transitions of care (VToC) for people hospitalized at University of California, San Diego Health reportedly reduced the 30-day readmission rates to 14.9%, which compares to a 20.1% 30-day readmission rate among those who received usual discharge care. The VToC clinic provided clinical management, medication reconciliation, primary care provider repatriation, and specialty care navigation. It was staffed by 12 hospitalists, two medical assistants, one pharmacist, and an on-demand interpreter service.
