Outpatient Follow-Up Within 30 Days Of Discharge Is Associated With Lower Risk Of Readmission

Post-discharge outpatient follow-up visits within 30 days reduced the risk of hospital readmission among older adults, according to the results of a recent study.

The analysis was based on 83 studies that reported 30-day all-cause readmissions for outpatient follow-up visits occurring within seven days, 14 days, and 30 days compared to readmissions for those who received no outpatient follow-up visit.

The studies included in the analysis were published between January 1, 2000, and August 4, 2025. Of the 83 studies reviewed, the meta-analysis included 76.
