Medicare Chronic Care Management Model To Test Technology-Supported Care Plus Outcomes-Aligned Payments
The Centers for Medicare & Medicaid Services (CMS) is preparing to launch the Medicare Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model to expand access to technology-supported care options that help beneficiaries prevent and manage chronic health conditions.
The model will also test outcome-aligned payments in which reimbursement for the participating provider organizations will be based on condition-specific clinical improvement or control targets compared to a baseline.
