In states awarded Medicaid Money Follows the Person (MFP) grants, the annual number of MFP transitions from institutional long-term services and supports (LTSS) to community living and home- and community-based services (HCBS) increased by 77% between January 2020 and December 2023, rising from 4,549 to 8,059, according to a December report from the Centers for Medicare & Medicaid Services (CMS).
