Minnesota Medicaid Audit Flags $1.7 Billion In Potentially Improper Payments
Minnesota Medicaid flagged an estimated $1.7 billion in potentially improper payments between January 2022 and October 2025, according to an addendum to Optum’s Program Integrity Vulnerability Assessment conducted for the Minnesota Department of Human Services (DHS). The findings identify payment vulnerabilities across 14 high-risk Medicaid service programs but do not represent confirmed determinations of fraud, waste, or abuse.
