State Medicaid Fraud Control Units (MFCUs) recovered $4.64 for every $1 spent on investigations in fiscal year 2025, generating nearly $2 billion in total recoveries, according to the U.S. Department of Health and Human Services Office of Inspector General (OIG).
Total recoveries reached nearly $2 billion, including $1.3 billion from criminal cases and $706 million from civil settlements and judgments.
