OPEN MINDS Announces 2026 Aging Services & Supports Summit To Focus On Innovative Care Models For Older Adults With Complex Needs 

Gettysburg, PA (May 2026) — OPEN MINDS announced the 2026 Aging Services & Supports Summit, a full-day program focused on care models for older adults with complex needs. The Summit will take place August 12, 2026, as part of the 2026 OPEN MINDS Service Excellence Institute in San Francisco, California and will address three key areas: technology-enabled independence, behavioral health access and social isolation, and care models designed for value-based markets. 

Older adults are living longer and in greater numbers than any generation before them. As the population grows, so does the complexity of need, including higher rates of chronic conditions, behavioral health challenges, and demand for services that support independence. Workforce shortages, funding changes, and rising acuity are contributing to gaps in care, driving the need for new technologies, payment models, and service approaches. 

“Every year without the infrastructure to support the growing population of older adults in the U.S. is a year when the gap widens between consumer need and market reality,” said Monica E. Oss, Chief Executive Officer of OPEN MINDS. “This summit is for the executives who decided that building now is the only viable strategy.” 

Summit sessions will highlight how organizations are responding to these challenges in practice. 

Leaders from Volunteers of America and the Institute for Public Health Innovation, joined by AARP’s former Chief Medical Officer, will examine how digital health tools, remote monitoring, and supportive technologies are being used in affordable senior housing communities to improve health and reduce isolation. 

Executives from Determined Health and Kaiser Permanente will discuss strategies to reduce stigma, expand access to behavioral health care, and address social isolation as a driver of declining health outcomes. 

The Summit will conclude with a session focused on how National Home Health Services and other organizations are aligning Medicare Advantage plans, provider organizations, hospitals, and community-based services to reduce avoidable hospitalizations and improve continuity of care. 

The summit will be chaired by Stuart Buttlaire, Ph.D., Vice President of Clinical Excellence & Leadership at OPEN MINDS, Michelle Matter, Chief Operating Officer of the San Diego State University Center for Excellence in Aging and Longevity, and Steve Hornberger, MSW, Co-Director of San Diego State University Center for Excellence in Aging and Longevity.
