Building Integrated, Whole Person Care Systems For Neurodiverse Populations: The Woods Case Study
Building Integrated, Whole Person Care Systems For Neurodiverse Populations: The Woods Case Study
September 14 @ 1:00 pm – 2:00 pm
More provider organizations serving individuals with intellectual and developmental disabilities (I/DD), autism, and other neurodiverse conditions are shifting to a “no wrong door” strategy that leverages integrated care models to address physical health, behavioral health, long-term services and supports, and social determinants of health. Done well, these programs can serve the most complex and costliest consumers and deliver great outcomes and cost-savings.
However, many executives find it challenging to determine where to start and how to implement whole person care in a sustainable manner.
During this one-hour live presentation, Tine Hansen-Turton, President and Chief Executive Officer at Woods System of Care (WSOC), and her colleagues, Sailaja Musunuri, Chief Medical and Clinical Officer, Executive Vice President of Integrative Medicine, and Executive Director of Mollie Woods Hare Global Center of Excellence; Dinetta Armstrong, Senior Vice President, Healthcare Management; and Oluwafunmike Adigun, Medical Director, will present the integrated care approach developed by WSOC, a national leader and innovator in whole person care.
These executives will draw on WSOC’s experience integrating primary care, behavioral health, nursing, community-based services, care coordination, and quality management to present a practical roadmap for organizations to build an integrated, coordinated, person-centered system of care.
This will include the organizational structures, workflows, interdisciplinary team practices, care coordination processes, population health strategies, and quality improvement methods that support successful integration.
