Hawaii Medicaid Launches Transformation Plan With Medical/Health Homes & Value-Based Purchasing
On February 27, 2014, the State of Hawaii launched an all-payer health care transformation plan that will feature strategies to redesign primary care, integrate behavioral health in primary care, and implement value-based purchasing. The goal is overall improved care coordination and streamlined service delivery. Key strategies include increasing enrollment in patient-centered medical homes (PCMHs), establishment of Medicaid health homes, and specialized programs for “super utilizers” (the small percentage of individuals with the highest utilization rates and costs).
