Patient-Centered Medical Home Program Led To Little Improvement In Quality & No Reduction In Use Of Services & Total Costs
Primary care practices participating in the Southeastern Pennsylvania Chronic Care Initiative (PACCI), a large multi-payer patient-centered medical home (PCMH) pilot, had little improvement in quality of care and no reduction in utilization or total costs. The PACCI pilot was conducted from June 1, 2008 to May 31, 2011; the outcomes for patients enrolled in the 32 PCMHs were compared to patients of 29 traditional primary care practices. Of 11 quality of care measures tracked, the PCMHs had significantly better performance on one measure related to nephropathy screening in diabetes.
