ApolloMed ACO Partners With Fresenius Medical & Boehringer Ingelheim Pharmaceuticals For Chronic Disease Management
Apollo Medical Holdings, Inc. (ApolloMed) has entered partnerships with a pharmaceutical company, Boehringer Ingelheim Pharmaceuticals, Inc. and specialty medical company Fresenius Medical Care for chronic disease management expertise for ApolloMed Accountable Care Organization (ACO) members. The partnerships are part of ApolloMed ACO’s high-risk program to target chronic and poly-chronic conditions. The partnership with Boehringer Ingelheim Pharmaceuticals, Inc. includes programs to manage the care of ApolloMed ACO patients with chronic obstructive pulmonary disease (COPD). The partnership with Fresenius Medical Care provides integrated specialized health care management for ACO patients with end-stage renal disease (ESRD).
