Kansas Medicaid Launches SMI Health Homes, Postpones Launch Of Chronic Conditions Health Homes
On July 1, 2014, Kansas Medicaid partially launched its health home program; enrollment opened for health homes for beneficiaries with serious mental illness (SMI), but has been delayed for chronic condition health homes. The chronic condition health homes will launch enrollment at an unspecified future date after January 2015.
The Kansas Medicaid health homes coordinate and manage enrollee care and access to Medicaid services through the KanCare Medicaid managed care organizations (MCOs), and the health homes provide supports and referrals for the person and their family.
