Hospital Guide To Reducing Medicaid Readmissions
In August 2014, the Agency for Healthcare Research and Quality (AHRQ) published a guide for hospitals to identify ways evidence-based strategies to reduce readmissions can be adapted or expanded to better address the transitional care needs of the adult Medicaid population. Adult Medicaid patients who are not hospitalized for childbirth experience readmission rates that are as high as or higher than those experienced by Medicare beneficiaries.
