This document is an August 2010 presentation to the Defense Centers of Excellence by the New Hampshire Department of Health and Human Services (DHHS) and the New Hampshire National Guard (NHNG) about the state’s Deployment Cycle Support Care Coordination Program (DCS-CCP) developed in 2007 in partnership with Easter Seals New Hampshire.
