Pursuing Financial Sustainability Of Person-Centered Care Models

On October 10, 2014 the SCAN Foundation and Avalere Health published a brochure entitled, Pursuing Financial Sustainability of Person-Centered Care Models. The authors identify the brochure as a “blueprint” for healthcare organizations to demonstrate and communicate the value of person-centered care models. Person-centered care has the following components: a targeted methodology that identifies the appropriate individuals to be studied; there must be an inter-disciplinary care team in place; and individual health and supportive services must be provided.
