California Medicaid Health Homes For Patients With Complex Needs (HHPCN) Draft Concept Paper
The California Department of Health Care Services (DHCS) has released a draft concept paper on a new Medicaid health home model, called Health Homes for Patients with Complex Needs (HHPCN). HHPCN will focus on frequent utilizers of health services and intensive care coordination for those with complex conditions or other conditions that make it difficult to manage health care services.
