A committee formed by the Institute of Medicine (IOM) recommended electronic health records (EHRs) collect standardized data for 12 social and behavioral health domains. EHRs already collect data for four of the recommended social and behavioral health domains (at specified recommended frequencies): race/ethnicity (at entry), tobacco use (screening and follow-up), alcohol use (screening and follow-up), and residential address (verify at every visit).

The IOM recommended that EHRs collect eight new data types, as follows:

Educational attainment--via two questions at entry
Financial resource strain--responses to screening and follow-up questions
