CMS Releases Medicare Medical Home Demonstration Care Management Reimbursement Rates

Per member per month care management fees will be as follows: Tier 1 practices, which provide basic MMHD care, will receive $27.12 for patients with a hierarchical condition categories (HCC) score lower than 1.6, and $80.25 for patients with higher scores, which indicate greater disease and future cost.
