OPEN MINDS On-Line News Senior Support Services A group of advocates for person-centered care in western New York have formed the Western New York Alliance for Person-Centered Care (Alliance) to develop a first in the nation, region-wide initiative to implement a culture change toward person-centered care at nursing home and assisted living facilities. The initiative is based on a desire to transform the areaÂ’s long-term care system. Of the 16,000 nursing homes nationwide, only about 250 have embraced culture change in a significant way.
