TransforMED to Help Blue Cross & Blue Shield of Kansas City Implement Patient-Centered Medical Home Pilot With 13 Primary Care Physician Groups

OPEN MINDS Weekly News Wire Strategic Health Care News On November 12, 2009, Blue Cross and Blue Shield of Kansas City in Missouri and the TransforMED consulting subsidiary of the American Academy of Family Physicians launched a two-year pilot of the patient-centered medical home model with 13 primary care physician groups in the region. The nearly 100 physicians in the 13 practices provide family medicine, internal medicine, and pediatrics.
