Humana & Dupage Medical Group Launch Outpatient Care Centers For Seniors With Complex Conditions In Chicago-Area
On January 2, 2013, Louisville-based Health plan company Humana, Inc. and Illinois-based provider DuPage Medical Group (DMG) announced they had launched two BreakThrough Care Centers, outpatient clinics designed to provide comprehensive outpatient services for older adults in the Chicago-area. The centers, which opened in Lombard and Naperville, will use RNs, health coaches, nutritionists, medication therapy management, physical therapists, and social services to health seniors manage their complex, chronic conditions.
