Oregon Health Care Provider Organizations Moving Toward Primary Care Medical Home Model

On January 12, 2012, the Oregon Health Authority announced that since August 2011, more than 80 health care clinics in the state have applied to be officially recognized by the state as patient-centered primary care homes (PCPCHs), and four clinics have gained PCPCH recognition—Mountainview Family Practice in Grants Pass, Hillsboro Pediatric Clinic in Hillsboro, Old Town Clinic in Portland, and OHSU Richmond Health Center in Portland.
