Walgreens & Greystone Healthcare Management Launch Collaborative Therapy Management Program In Florida To Cut Readmission Rates

On February 16, 2012, Walgreens and Greystone Healthcare Management launched a collaborative pharmacist-led therapy management pilot program in Florida to cut hospital readmit rates for patients in post-acute care settings. A key goal is increasing medication compliance rates. The pilot program will take place at Lady Lake Specialty Care, a 145-bed skilled nursing facility (SNF), in Lady Lake, Florida.
