Medicare Advantage Consumer Readmissions Drop To 8% When Pharmacists Visit Consumer Homes
A transition management pilot program focused on pharmacists visits to consumer homes following hospitalization of Medicare Advantage plan members resulted in a 36% decrease in the 30-day readmission rate, from 14% to 8%. The pilot deployed a pharmacist care manager to the consumer's home within 72 hours after discharge to conduct a thorough assessment of medications and other readmission risk factors.
