EHR-Based Predictive Risk Software & Targeted Interventions Cut Readmission Rates For Heart Failure by 19%
Ranking heart failure patients by risk level using an electronic health record (EHR) predictive modeling system system resulted in a sustained 19% reduction in readmissions for all heart failure patients over a 12-month period. The system, called the Piecesâ„¢ software system, was developed by PCCI. The system was tested in a prospective controlled trial of adult inpatients admitted to a large urban public teaching hospital for heart failure and two concurrent control conditions, heart attack and pneumonia.
