Aetna Launches Patient-Centered Medical Home For Oncology
On November 14, 2013, Aetna and Consultants in Medical Oncology and Hematology, PC (CMOH) launched a patient-centered medical home (PCMH) model for oncology for Aetna’s commercial, non-Medicare members. The model combines evidence-based decision support in cancer care, enhanced personalized services, and realigned payment structure to support treatment coordination. In the announcement, Aetna and CMOH noted that half of all new cancer patients are 65 or older, and most have one or more health condition in addition to cancer.
