Ontario, Canada Government Slows Growth Of Family Health Team Medical Home Model
On February 1, 2015, the Province of Ontario, Canada imposed limits on the number of family physicians who can participate in the province’s Family Health Team medical home model that features capitated rates and performance bonuses.
