Geisinger Health Plan ProvenHealth Navigator Medical Home Model Cut Costs By 8%
The Geisinger Health Plan "ProvenHealth Navigator" patient-centered medical home (PCMH) model reduced the plan’s Medicare claims costs for more than 6,400 members by 7.9% from January 1, 2006 through June 30, 2013. During the period, acute inpatient admission costs dropped by 18.7%, outpatient costs dropped by 5.1%, clinical professional costs dropped by 2.7%, and prescription costs dropped by 6.8%.
