PCMH Initiatives Increased To 160 In 2015, Up From From 26 In 2009

In 2015 there were more than 160 active patient-centered medical home (PCMH) initiatives; the PCMH initiatives are located across 48 states, Puerto Rico and the District of Columbia. The number of PCMH initiatives is up substantially from 26 PCMH initiatives in 18 states as of 2009. However, of the 160 initiatives, only about one-third provided adequate financial support; research suggests that a per member per month (PMPM) payment of between $6 to $8 is needed to sustain transformation.
