CMS Announces New CPC+Primary Care Model

On April 11, 2016, the Centers for Medicare & Medicaid Services (CMS) launched the multi-payer Comprehensive Primary Care Plus (CPC+) demonstration. CPC+ is a nationwide advanced primary care medical home model based on a CPC demonstration that began in 2012 with about 500 primary care provider organizations, and is slated to end during 2016. CPC+ is a five-year model that will begin in January 2017.
