Tennessee Medicaid To Implement Health Home Initiative In October 2016

Tennessee is preparing to implement Health Link, a Medicaid behavioral health home program, starting October 1, 2016. The behavioral health homes will receive a tiered per member per month (PMPM) rate ranging from $70 per member per month (PMPM) to $139 PMPM. The health homes will coordinate primary and behavioral health care for people with serious mental illness (SMI), and the PMPM rate includes targeted case management.
