HackensackAlliance Medicare ACO Attributes Savings To PCMH Model

The patient-centered medical home (PCMH) model helped the HackensackAlliance’s Medicare accountable care organization (ACO) generate savings for two consecutive years. In a comparison with other ACOs, the HackensackAlliance ACO had fewer hospital admissions, readmissions, and emergency department visits, which reduced the ACO’s expenses. The number of office visits exceeded those for the comparison ACOs.

The ACO’s utilization and savings data were as follows:
