HomeHero Partners With Cedars-Sinai Medical Center To Create Safe Transition Home Program To Transport Discharged Consumers From The Hospital To Their Homes

On June 23, 2016, Cedars-Sinai Medical Center and HomeHero a home care provider organization in Santa Monica, California launched the Safe Transition Home program. The program provides consumers transportation from the hospital to their homes and a readmission risk assessment, as well as a caregiver who will transport the consumer to follow-up appointments and procedures. The goal is to reduce missed post-discharge follow-up appointments, and ensure that the consumers arrive home safely.
