Illumination Foundation Intensive Care Coordination Program Saves $2.8 Million Over One Year For 38 Homeless Consumers

The Illumination Foundation’s Chronic Care Plus (CCP) initiative reduced health care costs by $2.8 million for 38 homeless individuals with high utilization of hospital emergency department care. During the year before entering the CCP program, their total medical costs were about $3.2 million. During the year after entering the CCP program, their total annualized costs were $514,336, for estimated annual savings of $2.8 million, or about $73,500 per participant.
