Connecticut Selects 9 Medicaid Provider Organizations For Shared Savings Program

The Connecticut Department of Social Services (DSS) selected nine primary care provider organizations on October 4 for negotiations to participate in a new state program. It’s called Person-Centered Medical Home Plus (PCMH+), and it’s a model to provide person-centered, comprehensive, and coordinated care. The contracts are slated to be finalized during January 2017.

The following organizations were offered the right to negotiate a PCMH+ contract:

Northeast Medical Group
Vincent’s Medical Group
Community Health Center Inc.
Cornell Scott-Hill Health Corporation
Fair Haven Community Health Clinic Inc.
Southwest Community Health Center
