Boston Public Health Commission Launches Partnership To House Vulnerable Populations

As of December 13, 2016, the Boston Public Health Commission (BPHC) is partnering with the Massachusetts Housing & Shelter Alliance, Beacon Health Options/Massachusetts Behavioral Health Partnership, and United Health Foundation to manage the Hospital to Housing initiative. This partnership aims to find permanent housing for homeless residents throughout the state who are frequently hospitalized for behavioral health issues. Funded by a $1.7 million partnership grant from United Health Group Foundation, this new initiative aims to reduce hospitalizations and improve health outcomes for members of this vulnerable population.
