Medical Home Network Expands Cook County Partnership To Include Mental Health Alerts
On December 13, 2016, Chicago-based Medical Home Network (MHN) announced that it had expanded its partnership with Illinois’ Cook County Health and Hospitals System (CCHHS) to include mental health and addiction treatment data. Specifically, the expansion allows primary care physicians in-network with CCHHS’ CountyCare Medicaid managed care health plan to receive real-time alerts when CountyCare members in their care receive care from local behavioral health provider organizations, immediate and urgent care centers, or CCHHS's Cermak Health Services, which provides services to 9,000 detainees within the Cook County Department of Corrections.
