Medicaid Health Homes: 21 States, 29 Models & 1.3 Million Consumers

As of January 1, 2017, there were 29 approved Medicaid health home models operating in 20 states and the District of Columbia. Some states submitted multiple health home state plan amendments (SPAs) for models to target different populations or conditions. More than 1 million Medicaid beneficiaries have enrolled in health homes, and nearly a dozen more states are planning to implement them.
