CMS Accountable Health Communities Model Selects 32 Participants To Serve As Local Test ‘Hubs’

On April 6, 2017, the Centers for Medicare & Medicaid Services (CMS) announced it had selected 32 organizations to participate in the five-year Accountable Health Communities model that will test three delivery approaches for linking clinical and community services.
