Between January 2013 through June 2015, the Maine Department of Health and Human Services (DHHS) failed to ensure that community-based organizations providing Medicaid home- and community-based services (HCBS) to adults with developmental disabilities reported all critical incidents to the state.  These were the finding of a federal review by Office of the Inspector General for the federal Department of Health and Human Services (HHS) released in August 2017.
