Health Homes Here To Stay: Q&A With Lori A. Accardi Catholic Charities Of Broome County/Encompass Health Home

When health and human service executives think about integrated care management, most of them envision health homes—a population-based integrated care service delivery model for individuals with chronic conditions and serious mental illness (SMI). In 2017, 1.3 million consumers were served by 33 health home programs in 22 states. This is a slight increase over 2016, when there were 20 states with 28 health home programs serving 1.2 million consumers (see U.S. Medicaid Health Home Market: The 2017 OPEN MINDS Update).
