Adirondack Health Institute & Unite Us Partner To Integrate Community Services & Improve Outcomes For New York State Medicaid Recipients

Unite Us and non-profit Adirondack Health Institute (AHI) are integrating their community services to improve health outcomes for Medicaid recipients in New York State. Under the partnership, AHI will implement the Unite Us software in order to support care coordination and collect of social determinants of health outcomes data from their health networks throughout northern New York.
