Amerigroup Texas & CareMore Health Launch A New Enhanced Primary Care Service For Medicaid Beneficiaries With Chronic Illness

On September 19, 2018, Amerigroup Texas and CareMore Health announced a collaboration to expand primary care options for Amerigroup Medicaid beneficiaries in the Dallas Fort Worth area who have chronic illness and complex health needs. Under this arrangement, CareMore Health opened a new care clinic that provides integrated primary care, chronic disease management, behavioral health, medication consultation, and assistance with identifying social service programs to address non-medical determinants of health.
