Minnesota’s ‘Return To Community Initiative’ Increased Nursing Home Community Discharge Rates By 11%
The Minnesota 'Return to Community Initiative' (RTCI) for newly admitted nursing home residents increased community discharge rates for the targeted population by an estimated 11.2%. The program effect increased with level of facility participation in RTCI.
The RTCI program was created in 2010 with the goal of helping people find the services and resources needed to stay in their home, rather than live in a nursing home over the long-term.
