Camden Coalition Intensive Care Coordination Program Effects On Hospital Admissions Limited
The Camden Coalition of Healthcare Providers’ Link2Care hotspotting program, which provides intensive care coordination to people with medically and socially complex conditions failed to significantly reduce the share of participants who were hospitalized again within 180 days. About 62.3% of the participants had another hospital admission within 180 days. Among a control group who received usual care, the 180-day admission rate was 61.7%.
