Care Coordination Accountability Measures For Primary Care Practice

In July 2011, the Agency for Agency for Healthcare Research and Quality (AHRQ) published a document introducing and establishing care coordination measures accountability measures for primary care practice as necessary and important assets to US health care delivery. The document also outlines: 1). Measures of care coordination processes as essential tools for generating evidence about care coordination and its outcomes; 2). Evaluating current practices; 3). Designing, implementing and assessing improvement activities; and 4). Supporting payment initiatives that target care coordination. Recommendations are presented for pediatric and adult populations from each of these perspectives, and alternatives are provided . . .
