During its first six months of operation after launching in the spring of-2020, the Horizon Neighbors In Health (HNIH) program operated by Horizon Blue Cross Blue Shield of New Jersey (Horizon BCBSNJ) screened 2,588 people for non-medical needs that were considered social determinants of health (SDOH). During this analysis, Horizon BCBSNJ found that the top needs were financial stability, basic health care, and food. Of the 2,588 screened in the first six months since HNIH launched, 28% (730 people) have graduated.
