SCAN Health Plan & Commonwealth Care Alliance Launch myPlace Health PACE Program

SCAN Health Plan and Commonwealth Care Alliance® (CCA) are launching a new Program of All-inclusive Care for the Elderly (PACE) integrated care delivery organization called myPlace Health. The partners will initially focus on launching myPlace Health in their current service areas; however, the specific markets have not been announced as of March 15, 2022.

PACE provides comprehensive medical and social services to Medicare and Medicaid beneficiaries age 55 and older who need nursing home-level care, but are still living in the community. Most PACE participants are dually eligible for both Medicare and Medicaid. The services include all Medicare . . .
